Phases-Primary Health Care for Women is an internal medicine clinic focused on promoting a woman’s healthy lifestyle and overall wellness through attentive medical care and patient education. We understand all women experience different phases in their lives that result in different medical needs – whether you are fresh out of school and needing a thorough check-up, developing headaches as a busy mom, struggling with weight gain or fatigue or you are post-menopausal and looking for an understanding place to talk about your specific health concerns.

Clinic Philosophy

Phases-Primary Health Care for Women serves patients in Madison and the surrounding areas, providing quality, personalized care using today’s latest technology and with physicians and nurse practitioners that understand the unique health needs of women.

Our practice recognizes that your time is valuable and your privacy is very important. As a consequence, we’ve designed our clinic so that you will only see our experienced physician and no other additional nurses or physicians in training.

Should your condition or health issue include the need for a gynecologist or an obstetrician, we are partnered with the highly-respected clinic, Physicians for Women, located in the main level of our building. Drs. Melius, Schurr, and Cardwell are available to provide immediate consultation and care.


Due to the nature of the health care industry, privacy laws and our respect for patients, each employee assumes an obligation to maintain a high level of confidentiality.

Patient information is held in strict confidence and only discussed with the individuals who need the information to perform their work. Each employee of Phases Primary Health Care for Women signs an employee statement of confidentiality form and receives training in HIPAA (Health Insurance Portability and Accountability Act of 1996) regulations and our practices’ own privacy policies.

  • Commercial Insurances (e.g. Allstate, United American Insurance, Hartford, most Medicare secondary insurances that are not HMO affiliated). We accept commercial insurances that give you the freedom to choose your physician or medical facility.

Should you desire services outside the covered benefits or health plan, we offer competitive cash rates. Please ask our team for further information.

"Point of Service" plans are not conventional HMO plans and usually cover a lower percentage of costs. Pre-authorization may be required by some plans to visit our clinic.


Co-payments are due on date of service. Please familiarize yourself with your individual insurance coverage so we may best provide care to you.

Please note: under the Affordable Health Care Act, preventative examinations do not require co-payment. It is important to review what constitutes a “preventative examination” according to your health insurance. This usually does not include addressing or treating medical concerns like it has in the past. Contact your insurance provider for clarification. You are welcome to call our office and we can help clarify based on your provider’s guidelines.

Insurance cards must be presented at each appointment. We accept cash, check, Visa, MasterCard, and Discover. Our practice accepts many forms of private health insurance. For any questions, please contact your insurance company directly or our business office at (608) 227-7007.

Financial Policy

Patient balances are due in full upon receipt of a current monthly statement.
